Food Service Plan Review
Food Service permit applicants and permit holders are required to submit the necessary documents to the health department for review and approval before construction or remodeling begins. The plan review process helps establishments save time and money by ensuring they will comply with current code and regulatory requirements. All establishments undergoing the plan review process must be inspected and approved by this Department prior to operation. Most food service operations are required to have a certified food manager before opening. Please note that plan review submittal instructions are included with the fee information of each category listed. Printed copies of materials can be picked up at our main health department location at 43525 Elizabeth Road in Mt. Clemens.
New Construction of a Food Service Establishment
Before construction of a new food service establishment begins, the permit applicant shall submit the following:
- Completed Plan Review Application
- Properly prepared Plan Review Worksheet. The Plan Review Manual will help you fill out the worksheet.
- Equipment specification sheets
- One (1) set of plans drawn to scale
- Proposed Menu
- Written Standard Operating Procedures
- Plan Review fee for a full plan review
Remodel of an Existing Food Service Establishment
Before work to remodel the food preparation, food handling, and food storage areas of an existing food service establishment begins or before changing any equipment, the permit holder shall submit the following:
- Completed Plan Review Application
- Properly prepared Plan Review Worksheet packet noting only the changes to be made. The Plan Review Manual will help you to fill out the worksheet.
- Equipment specification sheets for any new equipment
- One (1) set of plans drawn to scale
- Proposed Menu if changed
- Written Standard Operating Procedures
- Plan Review fee for a partial review
Extended Vacancy
Food service establishment has been closed six (6) months or longer and there are no changes to the layout or flow of the facility. The new owner must submit:
- Completed Extended Vacancy Application
- Plan Review Extended Vacancy Inspection fee
- Food Service License Application with fee based on seating
- Menu Draft (approval required before printing)
- Standard Operating Procedures
- Manufacturers Specification Sheets (for minor equipment changes/replacements only)
Site Review
A site review may be required for the installation of a piece of equipment that is more involved. Examples include the installation of a dishmachine or the replacement of hot water tank for a tankless hot water system. The permit holder must submit:
- Completed Site Review Application
- Plan Review Site Review Inspection fee
- Manufacturers Specification Sheets for any new equipment
Special Transitory Food Unit (STFU) / Mobile Food Establishment
An STFU is defined as a temporary food establishment that is licensed to operate throughout the state without the 14-day limits or a mobile food establishment that is not required to return to a commissary. A mobile is defined as a food establishment operating from a vehicle, including a watercraft, that returns to a mobile food establishment commissary for servicing and maintenance at least once every 24 hours. Food trucks will be licensed as one of these two options based on their operations. The applicant must submit the following:
- Completed STFU/Mobile Plan Review Application
- Plan Review fee for an STFU or
- Plan Review fee for a Mobile
- Equipment Specification Sheets
- One (1) set of plans drawn to scale
- Completed STFU/Mobile worksheet. The STFU/Mobile manual will help you fill out the worksheet.
- Mobiles will also need to submit a Commissary Verification Form and a routing slip.
All establishments undergoing the Plan Review process must be inspected and approved by this Department prior to operation.
