Seasonal Influenza (flu) Vaccine 2021-2022

It is recommended that all persons age 6 months and older receive a yearly flu vaccination. If you would like to schedule an appointment online, click the link below. Please call our Flu Vaccine Hotline: 586-466-7923 for more information.

Vaccine Registration and HIPAA Forms

Save time!  Please print and complete a registration form for each individual who is to receive a flu vaccination.  Child forms must be completed and signed by a parent or legal guardian.  Appointments are preferred. Walk-ins are accepted.

Cost

  • Medicare, Medicaid, and some commercial insurances are accepted
  • Cost for clients wishing to Self-Pay = $20
  • Clients younger than 19 years of age may be eligible to receive the flu vaccine at a lower cost through the Vaccines For Children (VFC) program. Details about this program can be found here

Insurance

Macomb County Health Department accepts the following insurances that often cover the full amount of the flu vaccination:

State and Federal Insurance: Medicare, Medicare Advantage, Medicaid, Medicaid Health Plans

Commercial Insurance: Blue Cross Blue Shield, Blue Care Network, Health Alliance Plan (HAP), McLaren Health Advantage, Priority Health, Total Health Care, TriCare

  • Refer to your insurance policy for full details regarding your coverage.
  • Bring insurance cards and photo ID to your appointment.
  • Clients whose insurance covers flu vaccination, but plan is not listed above, must contact their insurance providers for guidance if they wish to seek flu vaccination reimbursement.
  • All clients have the option to self-pay $20 for flu vaccination.

 

Please be Aware

  • Face masks are required to be worn at all flu vaccination clinic locations.
  • If your appointment is inside a health department clinic, only the person being vaccinated will be allowed in the building to maintain safe social distancing. Exceptions will be made for a parent or legal guardian who needs to accompany a minor or caregivers who need to accompany an adult needing assistance.
  • Reschedule your appointment if you are experiencing any symptoms of COVID‐19 (such as fever, cough, chills, etc.).
