Vaccine Registration and HIPAA Forms
Save time! Please print and complete a registration form for each individual who is to receive a flu vaccination. Child forms must be completed and signed by a parent or legal guardian. Appointments are preferred. Walk-ins are accepted.
A parent or guardian MUST be present for each child.

- Cost for clients wishing to Self-Pay = $20
- Clients younger than 19 years of age may be eligible to receive the flu vaccine at a lower cost through the Vaccines For Children (VFC) program. Details about this program can be found here

Insurance
Macomb County Health Department accepts the following insurances that often cover the full amount of the flu vaccination:
State and Federal Insurance: Medicare, Medicare Advantage, Medicaid, Medicaid Health Plans
Commercial Insurance: Blue Cross Blue Shield, Blue Care Network, Health Alliance Plan (HAP), McLaren Health Advantage, Priority Health, Total Health Care, TriCare
- Refer to your insurance policy for full details regarding your coverage.
- Bring insurance cards and photo ID to your appointment.
- Clients whose insurance covers flu vaccination, but plan is not listed above, must contact their insurance providers for guidance if they wish to seek flu vaccination reimbursement.
- All clients have the option to self-pay $20 for flu vaccination.
What to Bring:
- A photo identification card.
- Insurance cards (if applicable).
- If you are able, a Flu Vaccine Administration Record
- Note: Blank Forms will be available at the vaccine site if you do not have access to a printer.
