General Information
Scheduling an Appointment Online
- Clients interested in using the online system to schedule an appointment must complete the registration process. This includes providing your first and last name, date of birth, phone number, and email.
- At the end of the registration process, you are provided with a list of available appointment dates and times.
- If none of the listed appointments work for you, you may choose to receive a link and return to schedule your appointment at a later time.
Scheduling an Appointment via Telephone
- Appointments can also be scheduled by calling the Macomb County Health Department Immunization Program at 586-469-5372 or 586-465-8537, Monday through Friday from 8:30 AM to 5 PM.
Cost
- Medicare, Medicaid, and some commercial insurances are accepted (See insurance section below)
- Cost for clients wishing to Self-Pay = $20
- Clients younger than 19 years of age may be eligible to receive the flu vaccine at a lower cost through the Vaccines For Children (VFC) program. Details about this program can be found here

Macomb County Health Department accepts the following insurances that often cover the full amount of the flu vaccination:
- Medicare
- Medicare Advantage Plan
- Medicaid
- Medicaid Health Plan
- Blue Cross Blue Shield
- Blue Care Network
- Health Alliance Plan (HAP)
- McLaren Health Advantage
- Priority Health
- Total Health Care
- Tri Care
- Refer to your insurance policy for full details regarding your coverage.
- Bring insurance cards and Photo ID to your appointment.
- Clients whose insurance cover flu vaccination, but plan is not listed above, must contact their insurance providers for guidance if they wish to seek flu vaccination reimbursement.
- All clients have the option to self-pay $20 for flu vaccination.
Preparing for Your Vaccination Visit
What to Bring:
- A photo identification card.
- Insurance cards (if applicable).
- If you are able, a Flu Vaccine Administration Record (based on the client’s age):
- If Age 19 or older: Adult Flu Vaccine Administration Record
OR
- If Age 6 months through Age 18: Child Flu Vaccine Administration Record
Note: Blank Forms will be available at the vaccine site if you do not have access to a printer.
Please be aware:
- Face masks are required to be worn at all flu vaccination clinic locations.
- If your appointment is inside a health department clinic, only the person being vaccinated will be allowed in the building to maintain safe social distancing. Exceptions will be made for a parent or legal guardian who needs to accompany a minor or caregivers who need to accompany an adult needing assistance.
- Reschedule your appointment if you are experiencing any symptoms of COVID‐19 (such as fever, cough, chills, etc.).
