Hearing and Vision Program
The Hearing and Vision Program provides FREE hearing and vision screenings, referrals and follow-up for children in preschool through high school, ages 3-18 years.
This program is made up of 16 technicians, two clerical staff and a program manager. All technicians are comprehensively trained by Michigan Department of Health and Human Services (MDHHS) for both hearing and vision.
What Do I Need for Kindergarten Enrollment?
According to the Michigan Public Health Code (Act 368 of 1978), children entering kindergarten must have their hearing and vision tested before the first day of school.
Macomb County Health Department provides this service FREE of charge, by appointment ONLY, at various locations and dates throughout the school year. If your child was not screened in preschool, please call to schedule your appointment so your child will be prepared for kindergarten next fall. Appointments are currently being scheduled. If you have not arranged to have your child screened prior to the start of school, you will need to visit your doctor for this service.
The Hearing and Vision Health History Form is usually sent home by your preschool to fill out prior to the screening date. If you have misplaced the form, did not receive the form, or would like to fill it out prior to a screening appointment, please click here.
Important information to know:
- Please call the Macomb County Health Department Hearing and Vision Program at 586-412-5945 to schedule an appointment.
You will be notified of locations and appointment times when you call.
- DO NOT SCHEDULE AN APPOINTMENT FOR LOST OR MISPLACED PAPERWORK.
If lost or misplaced, please call the office for your options.
- For entrance into kindergarten, documentation is required and provided by Macomb County Health Department (click here to view sample).
Please put this document in a safe place until it is time for kindergarten registration.

Hearing Screenings
Hearing screenings are offered in schools to preschoolers and/or incoming kindergartners, as well as in grades K, 2, & 4.*

Vision screenings are offered in schools to preschoolers and/or incoming kindergarteners, as well as in grades 1, 3, 5, 7 & 9.*

Otology Clinics
An Otology Clinic is an additional service offered, free of charge, to any child who did not pass his/her regularly scheduled hearing screening. At an Otology Clinic, a child will be seen by an Ears, Nose and Throat (ENT) Physician to examine the ear. An Audiologist can also perform a comprehensive hearing test. Additional resources and/or referrals may be given to parents/guardians based on child’s results.
